State insurance consumer links (self-check)
Boundary
These links are public consumer pages so you can look up where to ask questions or file a complaint with a state insurance regulator—or find federal starting points. Your denial notice controls: use the address, portal, fax, and any deadline printed on that notice (and any later appeal decision). Those control over anything on this site.
DenialAppeal does not decide whether external review (or any next step) applies to you, does not calculate legal deadlines, and does not file for you. Plan and state rules vary. Many appeals are still denied. This is not legal advice.
DenialAppeal is a document-preparation tool, not a law firm. Using this page does not create an attorney–client relationship.
How to use this page
- Read your denial notice and plan documents first.
- If you need a state regulator consumer page, use the link for your state below—or the NAIC directory if your state is not listed.
- If coverage is through a self-funded employer plan, also read the self-funded note and the federal DOL link.
- Send any packet yourself using the notice instructions, or talk with a licensed attorney in your state.
Federal / hub
- Appeals overview (Marketplace / public appeals overview): How to appeal an insurance company decision — HealthCare.gov
- State insurance department directory (NAIC hub — not a .gov site; self-check only): NAIC — Insurance Departments
- Employment-based / self-funded starting point: DOL EBSA — ACA information for workers and families
If a link is missing for your situation, search your state department of insurance consumer site for “file a complaint” or “external review.” We do not maintain a 50-state directory. The NAIC list is an industry association directory for self-check—not a government website and not an official finding that any path applies to you.
State consumer pages (launch set)
One curated consumer entry per state. Missing from this list → use the NAIC hub or self-check search—not a made-up URL.
| State | Link |
|---|---|
| California | CDI — Consumer resources |
| Texas | TDI — Consumer information |
| New York | DFS — Information for consumers |
| Florida | DFS Division of Consumer Services |
| Illinois | DOI — Consumers |
| Pennsylvania | Insurance Department |
| Ohio | DOI — Information for consumers |
| Georgia | OCI — Consumer Services |
Self-funded employer plans
If you selected employer (self-funded): any further review path may use federal consumer channels rather than a state department of insurance. Confirm with HR or your plan materials. For a public starting point, see the DOL EBSA workers & families page. Links are for self-check only.
Not on this page
- No promise of a full 50-state directory
- No success rates or “likely to win” claims
- No statement that “your state gives you a right to external review” as a product conclusion
- No filing, mailing, or negotiating with your insurer on your behalf
- No legal advice or attorney–client relationship
